Independent investigations

What are they?

Independent investigations are required under national Health Service Guidance (94) 27. The responsibility for commissioning rests with strategic health authorities.

Independent investigations are held when a person commits murder or manslaughter and they have been subject to a formal programme of care with specialist mental health services in the six months prior to the event.  Investigations can also be commissioned where there is evidence of significant systematic service failure, such as a cluster of similar serious incidents.

The responsibility for commissioning rests with strategic health authorities.  Investigations are held following completion of any legal proceedings.
